Depending on the depth of your business requirements you may need some
    tables like this:

    DeptID
    Title
    LowSalary
    HighSalary

    Employees (employees with WC claims)
    -----------
    EmpID
    PosID
    First Name
    Last Name
    AvgWeeklyWage

InjuryTypes (8-10 broad classes of injuries, e.g., fall, strain, burn,
    puncture, etc.)
    -----------
    InjID
    InjDescription

    Incidents (injuries)
    -----------
    IncID
    EmpID
    InjID
    DateOfInjury
    LastDayWorked
    RTWDate
    ReserveAmnt
    PaidAmnt
    QuestionableFlag
    IncidentNotes

PayTypes (different payment types, e.g. Medical, Indemnity, Legal, Expense)
    -----------
    PayTypeID
    PayTypeDesc

Payments (payments made on a specific incident)
    -----------
    PayID
    IncID
    BillID
    PayTypeID
    PayAmnt
    PayDate
    PayDescription

Bills (bills received for a specific incident)
    -----------
    BillID
    PayID
    DateOfServiceStart
    DateOfServiceEnd
    ProviderID
    TotalCharges

Hope this gives you some ideas...
