I need an access database for problem resolution or corrective act

    Probably not, at least not one that would be configured to match your
    plant, supplier, and customer issues. This seems like quite a
    specialized database, not one that Microsoft would create a template
    for; tens of thousands of people will download a music recordings
    template, but there might be a dozen people in the world who need what
    you're describing.

    There are links to good tutorials, lists of books, etc. Probably far
    more Access databases are build from scratch, by designing a
    normalized set of tables, creating them, building forms for them, etc.
    than are ever built from templates; you'll get what YOU want.

    Or, consider hiring a professional, one who'll work closely with you
    to build a good, solid database that meets your specifications.

    I made a corrective action database app this year. It's not an
    uncommon requirement at ISO companies. If the OP posts back I'll send
    an email explaining the issues.
